En Paraguay, los datos sobre infecciones vaginales siguen siendo limitados. Esta falta de información dificulta el diseño de estrategias de prevención, diagnóstico precoz y tratamiento adecuado. Objetivo general: Determinar la prevalencia de infecciones vaginales en pobladoras del Departamento Central según rango etario, de 2024 a 2025. Materiales y métodos: Estudio observacional, descriptivo de corte transversal, con un enfoque prospectivo, entre 2024 y 2025. Se analizaron 80 muestras vaginales, recolectadas en el Departamento Central. Las muestras fueron obtenidas mediante hisopado vaginal estéril. Se remitieron a laboratorios de análisis clínicos. Para el análisis de datos se empleó Microsoft Excel. Se categorizaron según la clasificación de la edad adulta de la OMS. Resultados: De las muestras analizadas, 69% presentaba una infección monomicrobiana, y 24% padecían de coinfección, de las cuales todas contaban con presencia de Candida spp. La flora normal se observó en un 7,4% de las muestras. Se encontró una prevalencia de Gardnerella vaginalis en 43,75%, la prevalencia de Corynebacterium spp. fue de 22,5%, y la de Streptococcus gamma hemolítico fue de 16,25%. Candida spp. tuvo una frecuencia de 33,75%. En menores de edad se obtuvieron 2 muestras, ambas presentando infección. Hubo 58 adultas jóvenes, donde 96,5% tuvieron una flora anormal. Participaron 12 mujeres en adultez media, de las cuales 83,34% presentaron infección. Se obtuvieron 7 muestras de adultas mayores, donde el 100% presentó infección. Se obtuvo 1 muestra de una anciana, la cual presentó la flora normal. Conclusiones: Se evidenció una alta prevalencia de infecciones vaginales en el Departamento Central, con Gardnerella vaginalis como patógeno más frecuente, y una elevada proporción de coinfecciones. Se recomienda promover controles ginecológicos regulares, especialmente en jóvenes adultas y poblaciones con comorbilidades.<hr/>ABSTRACT Introduction: The female genital tract harbors a diverse, complex, and dynamic microbiome, the balance of which is essential for gynecological health. In Paraguay, data on vaginal infections remain limited. This lack of information hinders the development of prevention strategies, early diagnosis, and appropriate treatment. General Objective: Materials and Methods: An observational, descriptive, cross-sectional study with a prospective approach was conducted between 2024 and 2025. Eighty vaginal samples were analyzed, collected in the Central Department. Samples were obtained using sterile vaginal swabs and sent to clinical analysis laboratories. Data analysis was performed using Microsoft Excel. Participants were categorized according to the WHO classification of adulthood. Results: Of the analyzed samples, 69% had a monomicrobial infection, while 24% had co- infections, all of which included Candida spp. Normal flora was observed in 7.4% of the samples. The prevalence of Gardnerella vaginalis was 43.75%, Corynebacterium spp. was 22.5%, and gamma-hemolytic Streptococcus was 16.25%. Candida spp. had a frequency of 33.75%. Among minors, two samples were obtained, both showing infection. There were 58 young adult women, of whom 96.5% had abnormal flora. Twelve middle-aged women participated, with 83.34% presenting infection. Seven samples were obtained from older adult women, with 100% showing infection. One sample was obtained from an elderly woman, which exhibited normal flora. Conclusion: A high prevalence of vaginal infections was observed in the Central Department, with Gardnerella vaginalis as the most frequent pathogen and a high proportion of co-infections. It is recommended to promote regular gynecological check-ups, especially among young adult women and populations with comorbidities. <![CDATA[Treatment and evolution of peritonitis of appendiceal origin in patients under 18 years of age admitted to the Pediatrics service of the Hospital de Clínicas, San Lorenzo - Paraguay. Period 2021-2023]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1996-36962025000200010&lng=pt&nrm=iso&tlng=pt RESUMEN Introducción: La peritonitis apendicular es una de las principales emergencias quirúrgicas pediátricas, cuyo pronóstico depende del diagnóstico precoz y del tratamiento oportuno. Conocer su comportamiento epidemiológico local permite orientar intervenciones que mejoren los resultados clínicos. Objetivo: El objetivo del estudio fue describir la evolución clínica y las características epidemiológicas de la peritonitis apendicular en pacientes pediátricos hospitalizados en el Hospital de Clínicas entre 2021 y 2023. Materiales y Métodos: Estudio observacional, descriptivo, transversal y retrospectivo. Se incluyeron pacientes menores de 18 años con diagnóstico de peritonitis de origen apendicular. Se analizaron variables sociodemográficas, presentación clínica, tiempo de evolución, uso de antimicrobianos, complicaciones y estancia hospitalaria. Las variables cuantitativas se describieron mediante medias y desviación estándar, y las cualitativas mediante proporciones. Las asociaciones se evaluaron mediante la prueba de chi cuadrado. Resultados: Se incluyeron 85 pacientes; predominó el sexo masculino (68,2%) y el grupo etario de 10-14 años (42,4%). El 63,5% procedía del Departamento Central. El dolor abdominal fue el síntoma más frecuente (97,6%), seguido de vómitos (76,5%) y fiebre (54,1%). Más de la mitad consultó dentro de las primeras 24 horas (55,3%). El esquema antimicrobiano más utilizado fue la combinación de una cefalosporina de tercera generación con un nitroimidazol (85,9%). Las complicaciones ocurrieron en el 14,1% de los pacientes, destacándose sepsis (7,1%) y neumonía por contigüidad (5,9%). Tres pacientes requirieron cuidados intensivos (3,5%). El tiempo de evolución ≥48 horas se asoció significativamente con complicaciones (p &lt; 0,0001). Conclusión: La peritonitis apendicular afectó predominantemente a varones en edad escolar y adolescencia temprana. La demora en la consulta se asoció con mayor riesgo de complicaciones, resaltando la importancia del diagnóstico oportuno.<hr/>ABSTRACT Introduction: Appendiceal peritonitis is one of the main pediatric surgical emergencies, the prognosis of which depends on early diagnosis and timely treatment. Understanding its local epidemiological behavior allows for guiding interventions that improve clinical outcomes. Objective: The objective of this study was to describe the clinical evolution and epidemiological characteristics of appendiceal peritonitis in pediatric patients hospitalized at the Hospital de Clínicas between 2021 and 2023. Materials and Methods: Observational, descriptive, cross-sectional, and retrospective study. Patients under 18 years of age diagnosed with peritonitis of appendiceal origin were included. Sociodemographic variables, clinical presentation, duration of illness, antimicrobial use, complications, and length of hospital stay were analyzed. Quantitative variables were described using means and standard deviations, and qualitative variables using proportions. Associations were evaluated using the chi-square test. Results: 85 patients were included; Males predominated (68.2%), as did the 10-14 year age group (42.4%). 63.5% of patients came from the Central Department. Abdominal pain was the most frequent symptom (97.6%), followed by vomiting (76.5%) and fever (54.1%). More than half sought medical attention within the first 24 hours (55.3%). The most frequently used antimicrobial regimen was a combination of a third-generation cephalosporin and a nitroimidazole (85.9%). Complications occurred in 14.1% of patients, with sepsis (7.1%) and contiguous pneumonia (5.9%) being the most common. Three patients required intensive care (3.5%). A symptom duration of ≥48 hours was significantly associated with complications (p &lt; 0.0001). Conclusion: Appendiceal peritonitis predominantly affected school-aged males and young adolescents. Delay in seeking medical attention was associated with a higher risk of complications, highlighting the importance of timely diagnosis. <![CDATA[Microbiological profile of meningitis and encephalitis cases treated at the Central Hospital of the Social Security Institute, Paraguay, 2021-2024]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1996-36962025000200017&lng=pt&nrm=iso&tlng=pt RESUMEN Introducción: La meningitis y la encefalitis, representan urgencias médicas con alta morbimortalidad, especialmente en pacientes inmunocomprometidos. Objetivo: Describir el perfil microbiológico hallado en muestras de líquido cefalorraquídeo de pacientes con meningitis y encefalitis en el Instituto de Previsión Social durante el período comprendido entre el año 2021 al 2024. Metodología: Estudio observacional, descriptivo, retrospectivo que incluyó pacientes adultos de ambos sexos, con meningitis y/o encefalitis que fueron atendidos en el Hospital Central. Se excluyeron pacientes con abscesos cerebrales, infecciones posquirúrgicas y fichas con datos incompletos. Los datos fueron recolectados de la base de datos del Servicio de Laboratorio. Las variables fueron los aislamientos microbiológicos de muestras de líquido cefalorraquídeo y datos clínicos. Se utilizó el Software SPSS Statistics v.24® para análisis estadístico. Resultados: De 623 pacientes, 41 cumplieron criterios de inclusión, predominaron los aislamientos virales en el 65.8%(27), con predominio del Virus varicela zoster en 19.5%(8) dentro de ellos, pero el germen más frecuente fue el Streptococcus pneumoniae en 29.2%(26), luego Chikungunya en 14.6% (6), seguido de VHS-2 y M. tuberculosis en 9.8%(4) en cada caso, así como S. agalactiae y VHS-1 en 7.3% (3) en cada caso y en 2.4% (1) Virus herpes humano-7, Virus de Epstein-Barr, Parvovirus, Coxsackie virus, Echovirus, H. influenzae. Conclusión: Predominaron las etiologías virales sobre bacterianas, con un alto porcentaje de S. pneumoniae y Virus varicela zoster.<hr/>ABSTRACT Introduction: Meningitis and ancephalitis represent a high burden of global morbidity and mortality; therefore, early suspicion, prompt diagnosis, and appropriate treatment improve patient survival. Objective: To describe the microbiological profile found in cerebrospinal fluid samples from patients with central nervous system infections at the Instituto de Previsión Social during the period from 2021 to 2024. Methodology: This observational, descriptive, retrospective study included adult patients of both sexes with meningitis and/or encephalitis who were treated at the Central Hospital. Patients with brain abscesses, post-surgical infections, and records with incomplete data were excluded. Data were collected from the Laboratory Service database. The variables were microbiological isolates from cerebrospinal fluid samples and clinical data. SPSS Statistics v.24® software was used for statistical analysis. Results: Of the 623 patients, 41 met the inclusion criteria. Viral isolates predominated in 65.8% (27), with Varicella-zoster virus being the most frequent among them (19.5%, 8). However, the most common pathogen overall was Streptococcus pneumoniae (29.2%, 12), followed by Chikungunya virus (14.6%, 6), Herpes simplex virus type 2 and Mycobacterium tuberculosis (9.8%, 4 each), as well as Streptococcus agalactiae and Herpes simplex virus type 1 (7.3%, 3 each), and isolated cases (2.4%, 1) of Human herpes virus-7, Epstein-Barr virus, Parvovirus, Coxsackievirus, Echovirus, and Haemophilus influenzae. Conclusion: Viral etiologies predominated over bacterial ones, with a high proportion of Streptococcus pneumoniae and varicella-zoster virus identified. <![CDATA[Clinical and Epidemiological Characteristics of Patients with Nosocomial Infections Caused by <em>Enterococcus spp.</em> admitted to the Internal Medicine Department of the Hospital de Clinicas, 2022-2024]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1996-36962025000200026&lng=pt&nrm=iso&tlng=pt RESUMEN Las infecciones nosocomiales causadas por Enterococcus spp. representan una creciente amenaza para la salud pública debido a su resistencia a múltiples antimicrobianos. Este estudio tuvo como objetivo describir las características clínicas y epidemiológicas de los pacientes con cultivos positivos para Enterococcus spp. internados en el Servicio de Medicina Interna del Hospital de Clínicas, entre 2022 y 2024. Se realizó un estudio observacional, descriptivo, retrospectivo y transversal con 105 pacientes. Se analizaron variables como edad, sexo, comorbilidades, sitios de aislamiento, sensibilidad antimicrobiana y uso de dispositivos invasivos. Los resultados mostraron que Enterococcus faecium fue la especie más frecuente, con alta resistencia a la Vancomicina y la Ampicilina. Las infecciones del torrente sanguíneo fueron las más comunes, con una tasa de mortalidad del 28,5%. El 64,7% de los pacientes usaron sonda vesical y el 60,9% vía venosa central, con una duración promedio de 7,5 y 7,8 días, respectivamente. Este estudio destaca la relevancia de una vigilancia activa y estrategias de prevención para mitigar la propagación de infecciones nosocomiales resistentes.<hr/>ABSTRACT Nosocomial infections caused by Enterococcus spp. pose an increasing public health threat due to their resistance to multiple antimicrobial agents. This study aimed to describe the clinical and epidemiological characteristics of patients with positive Enterococcus spp. cultures hospitalized in the Internal Medicine Department at the Hospital de Clinicas between 2022 and 2024. A cross-sectional, retrospective, observational study was conducted, involving 105 patients. Variables such as age, sex, comorbidities, isolation sites, antimicrobial susceptibility, and use of invasive devices were analyzed. Results showed that Enterococcus faecium was the most frequent species, exhibiting high resistance to vancomycin and ampicillin. Bloodstream infections were the most common, with a mortality rate of 28.5%. Urinary catheters were used in 64.7% of patients, while central venous lines were used in 60.9%, with an average duration of 7.5 and 7.8 days, respectively. This study underscores the importance of active surveillance and preventive strategies to mitigate the spread of resistant nosocomial infections. <![CDATA[Socioconductual factors predict resistance to antituberculosis drugs in young and vulnerable populations]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1996-36962025000200035&lng=pt&nrm=iso&tlng=pt RESUMEN Introducción: La tuberculosis (TB) sigue siendo una emergencia global. El éxito de su tratamiento se ve comprometido por factores socioconductuales y la emergencia de resistencia a fármacos. Es imperativo cuantificar el impacto de estas variables, como el tabaquismo, en el riesgo de resultados adversos. Objetivo: Caracterizar el perfil sociodemográfico y clínico de pacientes con TB y determinar la asociación entre los factores socioconductuales predominantes y la resistencia al tratamiento. Metodología: Estudio observacional, descriptivo y analítico, de corte transversal, realizado en 106 pacientes consecutivos. Los datos fueron analizados mediante estadística descriptiva, Chi Cuadrado (chi^2) y Odds Ratio (OR). Resultados: La cohorte se caracterizó por ser de adultos jóvenes (89% entre 18 y 50 años) de procedencia urbana (90%) con un notorio bajo nivel educativo (77% con primaria incompleta o nula). Clínicamente, el 99% presentó localización pulmonar y la tasa de resistencia inicial fue del 5%. La prevalencia de factores conductuales fue alta (tabaquismo: 42%; adicciones: 43%). El análisis bivariado demostró una asociación estadísticamente significativa entre el tabaquismo y el riesgo de resistencia. El Odds Ratio fue de 5.85 IC 95%: p &lt; 0.05. Conclusión: La población con TB presenta una profunda vulnerabilidad sociodemográfica y un riesgo significativamente mayor de resistencia al tratamiento entre los pacientes fumadores. El control efectivo de la TB requiere urgentemente la integración del soporte psicosocial y estrategias de cesación tabáquica adaptadas al bajo nivel de escolaridad para reducir la emergencia de resistencia adquirida.<hr/>ABSTRACT Introduction: Tuberculosis (TB) remains a global emergency. Treatment success is compromised by socioconductual factors and the emergence of drug resistance. It is imperative to quantify the impact of variables like smoking on the risk of adverse outcomes. Objective: To characterize the sociodemographic and clinical profile of TB patients and to determine the association between predominant socioconductual factors and drug resistance. Methodology: An observational, descriptive, and analytical cross-sectional study was conducted on 106 consecutive patients. Data were analyzed using descriptive statistics, the Chi-Square test (chi^2), and the Odds Ratio (OR). Results: The cohort was characterized by young adults (89% between 18 and 50 years old) of urban origin (90%) with a notably low educational level (77% with incomplete primary education or less). Clinically, 99% presented pulmonary localization, and the initial resistance rate was 5%. The prevalence of behavioral factors was high (smoking: 42%; addictions: 43%). Bivariate analysis demonstrated a statistically significant association between smoking and the risk of resistance. The Odds Ratio was 5.85 (95% CI): (p &lt; 0.05). Conclusion: The TB patient population exhibits deep sociodemographic vulnerability and a significantly higher risk of treatment resistance among smokers. Effective TB control urgently requires the integration of psychosocial support and smoking cessation strategies adapted to the low educational level to reduce the emergence of acquired resistance. <![CDATA[Incidence and clinical characteristics of newborns with congenital syphilis before and during the SARS-COV-2 pandemic at the San Pablo General Maternal and Child Hospital]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1996-36962025000200042&lng=pt&nrm=iso&tlng=pt RESUMEN Introducción: La sífilis congénita ocurre por la transmisión transplacentaria de Treponema pallidum y se relaciona con desenlaces adversos en el embarazo, en gran medida prevenibles mediante diagnóstico y tratamiento oportuno. Objetivo: Identificar la incidencia y caracterizar clínicamente a recién nacidos con sífilis congénita en los periodos pre-pandémico y pandémico atendidos en el Hospital Materno Infantil San Pablo, entre los años 2018 a 2021. Metodología: Estudio observacional, descriptivo, retrospectivo, con enfoque cuantitativo. La técnica aplicada consistió en la revisión de historias clínicas de recién nacidos ingresados en el Hospital Materno Infantil San Pablo, en ambos periodos. Los datos recolectados como datos clínicos, de laboratorio fueron cargados y procesados con estadística descriptiva en el programa estadístico IBM Statistic versión 22 ®, los resultados se presentan en tablas y gráficos. Resultados: En el periodo prepandémico se identificaron 63 casos de sífilis congénita de un total de 7194 nacidos vivos (63/7194), con una incidencia de 8,7 por cada 1000 nacidos vivos y en el periodo pandémico se identificaron 86 casos de 8547 nacidos vivos (86/8547), lo que resultó en una incidencia de 10 por cada 1000 nacidos vivos. La frecuencia de recién nacidos sintomáticos se incrementó de 6,3% (4/63) en pre-pandemia a 20,9% (18/86) en pandemia. Rango etario de las madres entre 15 a 35 años, controles prenatales insuficientes Entre las manifestaciones clínicas en el periodo pandémico se observaron: pénfigo 3,5% (3), ragades 3,5% (3), rinitis serosanguinolenta 5,8% (5), hepatoesplenomegalia 7,0% (6), anemia 11,6% (10), linfadenopatía 3,5% (3) y neurosífilis 4,7% (4), en contraste con frecuencias menores al 5% en el periodo previo. Conclusiones: La incidencia fue mayor en la pandemia, con un incremento significativo de formas sintomáticas y hallazgos clínicos graves.<hr/>ABSTRACT Introdution: Congenital syphilis occurs through transplacental transmission of Treponema pallidum and is associated with adverse pregnancy outcomes, largely preventable through timely diagnosis and treatment. Objetive: To identify the incidence and clinically characterize newborns with congenital syphilis in the pre-pandemic and pandemic periods treated at the San Pablo Maternal and Child Hospital, between the years 2018 and 2021. Metodology: To identify the incidence and clinically characterize newborns with congenital syphilis during the pre-pandemic and pandemic periods treated at the San Pablo Maternal and Child Hospital, between the years 2018 and 2021. This was an observational, descriptive, retrospective study with a quantitative approach. The technique applied consisted of reviewing the medical records of newborns admitted to the San Pablo Maternal and Child Hospital during both periods. The collected clinical and laboratory data were entered and processed using descriptive statistics in the IBM Statistics version 22® statistical software. The results are presented in tables and graphs. Results: In the pre-pandemic period, 63 cases of congenital syphilis were identified out of a total of 7194 live births (63/7194), with an incidence of 8.7 per 1000 live births. In the pandemic period, 86 cases were identified out of 8547 live births (86/8547), resulting in an incidence of 10 per 1000 live births. The frequency of symptomatic newborns increased from 6.3% (4/63) in the pre-pandemic period to 20.9% (18/86) during the pandemic. Maternal age range between 15 and 35 years, insufficient prenatal care. Among the clinical manifestations observed during the pandemic period were: pemphigus 3.5% (3), ragades 3.5% (3), serosanguineous rhinitis 5.8% (5), hepatosplenomegaly 7.0% (6), anemia 11.6% (10), lymphadenopathy 3.5% (3) and neurosyphilis 4.7% (4), in contrast to frequencies of less than 5% in the previous period. Conclusions: The incidence of congenital syphilis was higher during the pandemic, with a significant increase in symptomatic forms and severe clinical findings. <![CDATA[Prevalence of <em>Staphylococcus aureus</em> in students and instructors of the Nursing program at the National University of Canindeyú]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1996-36962025000200051&lng=pt&nrm=iso&tlng=pt RESUMEN El Staphylococcus aureus es reconocido como uno de los principales agentes etiológicos de bacteriemia nosocomial en Norteamérica y Latinoamérica, representando un importante desafío en el control de infecciones hospitalarias. Objetivo: Describir la prevalencia de Staphylococcus aureus en estudiantes e instructores de prácticas hospitalarias de la carrera de Enfermería de la Facultad de Ciencias de la Salud. Metodología: Estudio cuantitativo, descriptivo, transversal y de campo. La recolección de muestras se llevó a cabo mediante hisopado nasal, seguido del cultivo en agar manitol para la identificación del microorganismo. Los datos fueron organizados y analizados mediante estadística descriptiva. Se cumplieron los principios éticos de investigación con seres humanos, garantizando consentimiento informado, confidencialidad y participación voluntaria. Resultados: De los 68 participantes, se encontró, la prevalencia global del 27,9% de portadores de S. aureus. Sin embargo, discriminados por curso se observó que en el segundo hubo una prevalencia del 30% con mayor portación en mujeres y sin casos positivos en varones, en el tercero el 25,8% donde la portación fue más frecuente en hombres que en mujeres y el cuarto curso presentó la prevalencia más alta 33,3% con valores similares entre ambos sexos. Entre los instructores, la prevalencia fue del 22,2%, con mayor presencia en varones que en mujeres. Discusión: La elevada prevalencia de S. aureus en esta población resulta epidemiológicamente relevante, considerando que las prácticas de enfermería implican contacto directo y frecuente con pacientes vulnerables, lo que aumenta el riesgo de transmisión y la posibilidad de infecciones asociadas a la atención de salud.<hr/>ABSTRACT Staphylococcus aureus is recognized as one of the main etiological agents of nosocomial bacteremia in North and Latin America, representing a significant challenge in the control of hospital infections. Objective: To describe the prevalence of Staphylococcus aureus in nursing students and instructors during their clinical rotations at the Faculty of Health Sciences. Methodology: A quantitative, descriptive, cross-sectional, and field study was conducted. Samples were collected via nasal swab, followed by culture on mannitol agar for microorganism identification. Data were organized and analyzed using descriptive statistics. Ethical principles for research involving human subjects were followed, ensuring informed consent, confidentiality, and voluntary participation. Results: Of the 68 participants, an overall prevalence of S. aureus carriers was found to be 27.9%. However, when broken down by year, it was observed that the second year had a prevalence of 30%, with higher rates of carriage in women and no positive cases in men; the third year had a prevalence of 25.8%, with carriage more frequent in men than in women; and the fourth year presented the highest prevalence at 33.3%, with similar values between both sexes. Among instructors, the prevalence was 22.2%, with a higher presence in men than in women. Discussion: The high prevalence of S. aureus in this population is epidemiologically relevant, considering that nursing practices involve direct and frequent contact with vulnerable patients, which increases the risk of transmission and the possibility of healthcare-associated infections. <![CDATA[Disseminated cutaneous leishmaniasis. Además de la forma visceral, puede presentarse como forma cutánea, mucocutánea y en forma diseminada. La hipersensibilidad y funcionalidad de los neutrófilos juegan un papel importante en el desarrollo de la última forma de presentación. Responden bien al tratamiento convencional. Se presenta el caso de un adulto varón, con respuesta al tratamiento con anfotericina B.<hr/>ABSTRACT Leishmaniasis is a parasitic disease of tropical areas, which affects skin, mucous membranes and viscera. In addition to the visceral form, it can present as cutaneous, mucocutaneous and disseminated forms. Hypersensitivity and functionality of neutrophils play an important role in the development of the latter form of presentation. They respond well to conventional treatment. We present the case of an adult male, who responded to treatment with amphotericin B.
