Objetivo: Determinar el manejo del cáncer de esófago en pacientes internados en la Primera Cátedra de Clínica Quirúrgica del Hospital de Clínicas San Lorenzo del 2010 al 2022. Material y método: Estudio observacional, retrospectivo, descriptivo y transversal. Resultados: 31 pacientes con diagnóstico de cáncer de esófago en Primera Cátedra de Clínica Quirúrgica del Hospital de Clínicas durante el período de 2010 a 2022. El 83,9% de los pacientes eran hombres. Media de edad, 63,7 años. La manifestación clínica más frecuente fue la disfagia y pérdida de peso con síndrome constitucional. Presentaron lesiones vegetantes 41,9%. Las lesiones estaban ubicadas en tercio medio 67%. El tipo histológico más frecuente en 83,9% fue carcinoma escamoso. Conclusión: El desafío actual es implementar políticas de screening de esta enfermedad teniendo en cuenta los factores de riesgo asociados.<hr/>Abstract Introduction: Esophageal cancer is one of the most aggressive neoplasms of the digestive tract, with high morbidity and mortality, and is the eighth most common cancer worldwide. Objective: To determine the management of esophageal cancer in patients admitted to the First Department of Clinical Surgery at the San Lorenzo Clinics Hospital from 2010 to 2022. Materials and methods: An observational, retrospective, descriptive, and cross-sectional study. Results: Thirty-one patients were diagnosed with esophageal cancer at the First Department of Clinical Surgery at the San Lorenzo Clinics Hospital from 2010 to 2022. 83.9% of the patients were men. Mean age was 63.7 years. The most frequent clinical manifestation was dysphagia and weight loss with constitutional syndrome. 41.9% presented vegetative lesions. 67% of the lesions were located in the middle third. The most common histological type, 83.9%, was squamous cell carcinoma. Conclusion: The current challenge is to implement screening policies for this disease, taking into account the associated risk factors. <![CDATA[Technique of reinforcement for the abdominal wall with longitudinal suture lateral to the incision (reiforced tension line-RTL) for midline closure in order to prevent evisceratio]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S2307-04202025000100012&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La evisceración es una complicación posterior a una laparotomía la técnica de RTL puede prevenirla. Objetivo: Describir resultados utilizando la técnica de RTL para prevención de evisceración en pacientes de riesgo. Material y método: Estudio prospectivo, observacional descriptivo, de corte transverso en paciente con factores de riesgo de evisceración con puntaje mayor a 6 de la escala Van Ramshorst utilizando técnica de RTL para cierre de la línea media de marzo de 2023 a febrero de 2024, en la IICCQ Hospital de Clínicas FCM-UNA. Resultados: se estudiaron 30 pacientes con 6 o más factores de riesgo para evisceración según escala de Van Ramshorst. Factores de riesgo encontrados: edad, tipo de cirugía, anemia, ascitis, tos. Edad promedio fue 55,2 ± 10,3 años, incisión mayor a 18 cm (63%) de los pacientes, tiempo quirúrgico fue de 180 ± 165 minutos (50%), infección del sitio quirúrgico (20%), 1(3,3%) óbito por sepsis abdominal. Se presentaron 2 (6,7%) Conclusión: La técnica RTL es una técnica segura, rápida, fácil de realizar, puede prevenir evisceraciones.<hr/>Abstract Introduction: Evisceration is a complication after a laparotomy, the RTL technique can prevent it. Objective: To describe results using the RTL technique for the prevention of evisceration in at-risk patients. Material and method: Prospective, observational, descriptive, cross-sectional study in a patient with risk factors for evisceration with a score greater than 6 on the Van Ramshorst scale using RTL technique for midline closure from March 2023 to February 2024, at the IICCQ Hospital de Clínicas FCM-UNA. Results: 30 patients with 6 or more risk factors for evisceration according to the Van Ramshorst scale were studied. Risk factors found: age, type of surgery, anemia, ascites, cough. Mean age was 55.2 ± 10.3 years, incision greater than 18 cm (63%) of patients, surgical time was 180 ± 165 minutes (50%), surgical site infection (20%), 1 (3.3%) death due to abdominal sepsis. 2 (6.7%). Conclusion: The RTL technique is a safe, fast, easy to perform technique, it can prevent eviscerations.<hr/>Keywords: evisceration, midline closure <![CDATA[Hepatic abscess secondary to exceptional foreign body]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S2307-04202025000100016&lng=en&nrm=iso&tlng=en RESUMEN Se presenta una serie de tres casos de abscesos hepáticos secundarios a la ingestión de cuerpos extraños. La presentación clínica de esta entidad es variable, lo que dificulta su diagnóstico oportuno. Un alto índice de sospecha clínica, el uso de estudios de imagen y, en algunos casos, la endoscopia, son cruciales para identificar la causa subyacente del absceso. En todos los casos recibieron tratamiento con antibiótico. El tratamiento definitivo requirió drenaje quirúrgico y extracción del cuerpo extraño en dos de los tres casos, con resultados postoperatorios favorables. Y, en uno, se resolvió con Endoscopía Digestiva Alta (EDA) y Punción Aspirativa con Aguja Fina (PAAF) de forma exitosa. Esta serie de casos destaca la importancia de considerar la ingestión de cuerpos extraños en pacientes con absceso hepático, incluso sin antecedentes claros, para asegurar un manejo adecuado.<hr/>Abstract A series of three cases of hepatic abscesses secondary to foreign body ingestion is presented. The clinical presentation of this condition is variable, which complicates timely diagnosis. A high level of clinical suspicion, imaging studies, and in some instances, endoscopy, are essential to identify the underlying cause of the abscess. All cases received antibiotic treatment. Definitive treatment required surgical drainage and foreign body extraction in two of the three cases, with favorable postoperative outcomes. In one case, resolution was achieved through Upper Digestive Endoscopy (EDA) and Fine Needle Aspiration (FNA-US) successfully. This case series emphasizes the importance of considering foreign body ingestion in patients with hepatic abscesses, even in the absence of clear histories, to ensure proper management. <![CDATA[Acute pancreatitis and retroperitoneal perforation after endoscopic retrograde cholangiopancreatography: case report]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S2307-04202025000100020&lng=en&nrm=iso&tlng=en RESUMEN Paciente de 54 años sometida a Colangiopancreatografía Retrógrada Endoscópica (CPRE) por coledocolitiasis, que desarrolló pancreatitis aguda biliar severa y perforación tipo II retroperitoneal. El diagnóstico se confirmó por imagen y se manejó con drenaje percutáneo y tratamiento conservador, con buena evolución clínica. El caso destaca la importancia del reconocimiento precoz de complicaciones graves post-CPRE y el éxito del manejo no quirúrgico.<hr/>Abstract A 54-year-old patient underwent ERCP for choledocholithiasis and developed severe acute biliary pancreatitis and a type II retroperitoneal perforation. Diagnosis was confirmed by imaging and managed with percutaneous drainage and conservative treatment, with good clinical evolution. This case highlights the importance of early recognition of serious ERCP complications and the success of non-surgical management. <![CDATA[Paraguayan Consensus regarding diagnosis and treatment of acute pancreatitis in adults. Guide for surgeons - 2025]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S2307-04202025000100024&lng=en&nrm=iso&tlng=en RESUMEN Este documento detalla el consenso generado por expertos paraguayos en el manejo de la pancreatitis aguda (PA). El objetivo es unificar criterios de diagnóstico y tratamiento basados en la evidencia científica, adaptada al contexto local, buscando mejorar la calidad de la atención y promover la investigación en esta área. La metodología empleada incluyó la selección de 24 expertos a través de una encuesta nacional, quienes se organizaron en tres mesas de trabajo para abordar diferentes aspectos de la PA. Se realizó una revisión exhaustiva de la literatura científica en bases a publicaciones que datan de 1990 a 2024. El documento define la PA como un proceso inflamatorio agudo del páncreas, con la litiasis biliar y el alcohol como principales causas. La gravedad es clasificada en leve, moderada, grave y crítica, basándose en la presencia de necrosis, falla orgánica y complicaciones. Se destaca la importancia de los parámetros clínicos, laboratorio (lipasa y amilasa) y los estudios de imágenes. Se recomienda el uso de la clasificación de Petrov o Atlanta revisada para establecer la gravedad de la PA y la clasificación de Marshall para determinar las complicaciones sistémicas. La TAC es considerada fundamental para la clasificación de gravedad y el diagnóstico de complicaciones, especialmente entre el tercer y décimo día de evolución. Se enfatiza la utilidad de la ecografía como herramienta inicial para identificar la causa biliar y detectar complicaciones tempranas. Se busca - finalmente - estandarizar el manejo de la PA y mejorar los resultados en los pacientes con esta patología en el Paraguay.<hr/>Abstract This document outlines the consensus reached by Paraguayan experts on the handling of acute pancreatitis (AP). The objective is to unify diagnostic and treatment criteria based on scientific evidence adapted to the local context, aiming to improve the quality of care and promote research in this field. The methodology involved selecting 24 experts through a national survey, who were then organized into three working groups to address different aspects of AP. A comprehensive review of the scientific literature was conducted, covering publications from 1990 to 2024. The document defines AP as an acute inflammatory process of the pancreas, with biliary lithiasis and alcohol consumption as the leading causes. Severity is classified as mild, moderate, severe, or critical, based on the presence of necrosis, organ failure, and complications. It highlights the importance of clinical parameters, laboratory tests (lipase and amylase), and imaging studies. Use of the Petrov or revised Atlanta classification is recommended to assess the severity of AP, along with the Marshall scoring system to evaluate systemic complications. CT scanning is considered essential for grading severity and diagnosing complications, particularly between the third and tenth day of disease progression. Ultrasound is emphasized as a valuable initial tool for identifying biliary causes and detecting early complications. Ultimately, the goal is to standardize the handling of AP and improve patient outcomes with this pathology in Paraguay.
