El mantenimiento intravenoso (IV) y los fluidos no destinados a la reanimación son fuentes acumulativas subestimadas en la fase de estabilización. Objetivo: Evaluar la asociación entre la acumulación de fluidos IV, la disfunción orgánica múltiple y la mortalidad en cuatro puntos temporales. Materiales y Métodos: Cohorte observacional retrospectiva unicéntrica censal (n=72). Pacientes de 1 mes a 18 años con shock séptico (PHOENIX-8 y SLACIP), UCIP del Hospital de Clínicas, FCM-UNA, Paraguay, enero-diciembre 2025. SL% = [(ingresos − egresos) acumulados (L) / peso al ingreso (kg)] × 100 (fórmula de Goldstein); como el balance se expresó en mL/kg, equivale a balance neto acumulado (mL/kg) ÷ 10. Análisis bivariado: Mann-Whitney U, chi-cuadrado/Fisher, OR con IC 95% (corrección de Haldane). Análisis de sensibilidad: OR ajustado por PRISM III. Curva ROC. Correlaciones de Spearman. Resultados: Mortalidad 22,2% (16/72). Toda la mortalidad se observó en el grupo con SL%≥10% al Día 3 (OR 29,79; IC 95%: 1,70-522; p&lt;0,001). El OR ajustado por PRISM III: 18,4 (IC 95%: 1,1-312; p=0,044). AUC-ROC del SL%D3: 0,87 (IC 95%: 0,78-0,96; corte óptimo 10,5%). Mantenimiento IV mayor en fallecidos desde el Día 1. Estadios KDIGO 2-3 se observaron exclusivamente en fallecidos. Conclusiones: La SL%≥10% al Día 3 es un marcador pronóstico asociado a mortalidad y disfunción orgánica múltiple. El diseño observacional no permite inferir causalidad.<hr/>ABSTRACT Introduction: Fluid overload (FO) is a frequent complication of pediatric septic shock. Maintenance Intravenous (IV) fluids and fluids not intended for resuscitation are underestimated cumulative sources of fluid overload during the stabilization phase. Objective: To evaluate the association between IV fluid accumulation, multiple organ dysfunction, and mortality at four time points. Materials and Methods: This was a single-center, census-based, retrospective observational cohort study (n=72). We included patients aged 1 month to 18 years with septic shock (PHOENIX-8 and SLACIP), admitted to the PICU of the Hospital de Clínicas, FCM-UNA, Paraguay, during January-December 2025. Variables were SL% = [(cumulative intakes − outputs) (L) / weight on admission (kg)] × 100 (Goldstein formula); since the balance was expressed in mL/kg, it is equivalent to cumulative net balance (mL/kg) ÷ 10. We performed bivariate analysis: Mann-Whitney U, chi-square/Fisher, OR with 95% CI (Haldane correction). Sensitivity analysis: OR adjusted by PRISM III. ROC curve. Spearman correlations. Results: Mortality was 22.2% (16/72). All mortality was observed in the group with SL%≥10% on Day 3 (OR 29.79; 95% CI: 1.70-522; p&lt;0.001). The PRISM III adjusted OR was 18.4 (95% CI: 1.1-312; p=0.044). The AUC-ROC for SL% on D3 was 0.87 (95% CI: 0.78-0.96; optimal cutoff 10.5%). IV maintenance was more frequent in patients who died from Day 1 onward. KDIGO stages 2-3 were observed exclusively in deceased patients. Conclusions: SL%≥10% on Day 3 is a prognostic marker associated with mortality and multiple organ dysfunction. The observational design does not allow for inferences of causality. <![CDATA[Intraoperative analgesia and postoperative pain in pediatric patients undergoing outpatient surgery. Prospective observational cohort follow-up study]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200139&lng=en&nrm=iso&tlng=en Resumen Introducción: El control inadecuado del dolor postoperatorio puede comprometer el confort del paciente y retrasar el alta. Objetivo: Analizar la asociación entre el dolor en el postoperatorio con el esquema de analgesia intraoperatoria, la especialidad quirúrgica y la duración de la cirugía, en pacientes pediátricos sometidos a cirugía ambulatoria. Materiales y Métodos: Estudio observacional, prospectivo de una cohorte, realizado en septiembre y noviembre de 2025 en un hospital pediátrico. Por muestreo a conveniencia se incluyeron, pacientes de 0 a 18 años sometidos a cirugía ambulatoria. Variables: demográficas, tipo y duración de la cirugía, especialidad quirúrgica, esquema de analgesia intraoperatoria, dolor postoperatorio, tiempo de estadía y hospitalización. El análisis fue realizado en SPSS v21 utilizando estadísticas descriptivas e inferenciales. El protocolo fue aprobado por el comité de ética con consentimiento informado. Resultados: Ingresaron 127 pacientes, con una edad mediana de 6 años (p25-p75 3-10); el 72,4% sexo masculino, 60,6% recibió anestesia general con bloqueo loco regional y antiinflamatorio no esteroideo (AG+B+AINE). A la hora del postoperatorio, el 85,8% se encontraba sin dolor, asociado con el esquema AG+B+AINE (p = 0.0006). La duración del procedimiento no se asoció con la presencia de dolor. El tiempo de estadía fue de 7,5 horas. Ningún paciente se hospitalización. Conclusión: La mayoría no presento dolor en el post operatorio. La ausencia de dolor se asoció con el esquema AG+B+AINE. No se encontró asociación entre el dolor y la duración de la cirugía. El tiempo de estadía fue breve. Ningún paciente se hospitalizo.<hr/>ABSTRACT Introduction: Inadequate postoperative pain control can compromise patient comfort and delay discharge. Objective: To analyze the association between postoperative pain with the intraoperative analgesia scheme, surgical specialty and duration of surgery, in pediatric patients undergoing outpatient surgery. Materials and Methods: This was an observational, prospective cohort study, carried out in September and November 2025 in a pediatric hospital. By convenience sampling, patients aged 0 to 18 years undergoing outpatient surgery were included. Variables: demographics, type and duration of surgery, surgical specialty, intraoperative analgesia scheme, postoperative pain, length of stay and hospitalization. The analysis was performed in SPSS v21 using descriptive and inferential statistics. The protocol was approved by the ethics committee with informed consent. Results: 127 patients were admitted, with a median age of 6 years (p25-p75 3-10); 72.4% were male, 60.6% received general anesthesia with regional loco blockade and non-steroidal anti-inflammatory drug (AG+B+NSAID). At the postoperative hour, 85.8% were pain-free, associated with the AG+B+NSAID scheme (p = 0.0006). The duration of the procedure was not associated with the presence of pain. The stay time was 7.5 hours. No patient was hospitalized. Conclusion: The majority did not present pain in the postoperative period. The absence of pain was associated with the AG+B+NSAID regimen. No association was found between pain and duration of surgery. The length of stay was short. No patient was hospitalized. <![CDATA[Survey of healthcare professionals’ knowledge and attitudes regarding RSV and its prevention]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200150&lng=en&nrm=iso&tlng=en RESUMEN Introducción: En 2025, Paraguay implementó Nirsevimab para la prevención del virus sincitial respiratorio (VSR) en lactantes nacidos entre enero y julio. Objetivo: Evaluar los conocimientos y actitudes de los profesionales de la salud (PS) sobre el VSR y su prevención. Materiales y métodos: Estudio observacional, transversal y descriptivo mediante una encuesta CAP autoadministrada (Google Forms), dirigida a PS. El cuestionario incluyó preguntas sobre conocimientos, percepción de riesgo y actitudes frente al Nirsevimab. Los datos se analizaron con estadística descriptiva. Resultados: Respondieron 774 profesionales. El 81% conocía el grupo de mayor riesgo de hospitalización por VSR, pero solo el 42% reconocía el riesgo en lactantes previamente sanos. El 85% conocía el Nirsevimab, el 83% la población objetivo y el 55% su efectividad. El 83% identificó ventajas frente a Palivizumab y el 89% recomendaría su administración, principalmente por la evidencia de eficacia y seguridad (80%). Las principales barreras percibidas fueron la logística de distribución (78%), la preparación del sistema de salud (76%), el desconocimiento de los padres (76%) y de los profesionales (61%). El 71% consideró que Nirsevimab reducirá las hospitalizaciones por VSR. Conclusiones: Los profesionales mostraron conocimientos adecuados sobre aspectos generales del VSR y Nirsevimab, aunque persistieron vacíos en recomendaciones específicas. Existe una alta aceptación de la estrategia preventiva, pero se identifican barreras relacionadas con capacitación, información a las familias y aspectos logísticos que deben fortalecerse para optimizar su implementación.<hr/>ABSTRACT Introduction: In 2025, Paraguay implemented nirsevimab for the prevention of respiratory syncytial virus (RSV) infection in infants born between January and July. Objective: To assess healthcare professionals’ (HCPs’) knowledge and attitudes regarding RSV and its prevention. Materials and Methods: A descriptive, cross-sectional observational study was conducted using a self-administered Knowledge, Attitudes, and Practices (KAP) survey distributed via Google Forms to HCPs. The questionnaire included questions assessing knowledge, risk perception, and attitudes toward nirsevimab. Data were analyzed using descriptive statistics. Results: A total of 774 healthcare professionals responded. Eighty-one percent were aware of the population at highest risk of RSV-related hospitalization; however, only 42% recognized the risk among previously healthy infants. Eighty-five percent were familiar with nirsevimab, 83% correctly identified the target population, and 55% were aware of its effectiveness. Eighty-three percent identified advantages of nirsevimab over palivizumab, and 89% would recommend its administration, primarily based on evidence of efficacy and safety (80%). The main perceived barriers were distribution logistics (78%), health system preparedness (76%), lack of knowledge among parents (76%), and lack of knowledge among healthcare professionals (61%). Seventy-one percent believed that nirsevimab would reduce RSV-related hospitalizations. Conclusions: Healthcare professionals demonstrated adequate knowledge of general aspects of RSV and nirsevimab, although gaps remained regarding specific recommendations. There was high acceptance of the preventive strategy; however, barriers related to training, family education, and logistical considerations were identified and should be addressed to optimize its implementation. <![CDATA[Knowledge and practices of medical students regarding community-based management and prevention of bronchiolitis in children under two years of age]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200163&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La bronquiolitis es una enfermedad respiratoria viral, causada por el Virus Respiratorio Sincitial (VRS), más comúnmente en niños menores de 2 años. El objetivo de este estudio es: Determinar el conocimiento y prácticas en el manejo comunitario y prevención de la bronquiolitis en menores de 2 años, de los estudiantes de Medicina. Material y Métodos: Estudio observacional, cuantitativo, descriptivo, y de corte transversal. La muestra fue de 92 estudiantes de medicina del cuarto y quinto año, Universidad Centro Médico Bautista. Asunción, Paraguay, durante diciembre de 2025 y primer trimestre 2026. Mediante una encuesta, previo consentimiento informado, se aplicó un formulario de Google Forms, con una escala de Likert. Las puntuaciones obtenidas por los estudiantes presentaron una media global de 3,91 (DE = 0,42; rango observado: 2,00-5,00). La consistencia interna del cuestionario, estimada mediante el coeficiente alfa de Cronbach en la muestra estudiada, fue de 0,799. Resultados: El nivel de conocimiento es alto en las dimensiones: Signos y síntomas, cuidados domiciliarios, soporte terapéutico y prevención. Las medidas de higiene y control ambiental bordean la unanimidad. En cuanto al soporte terapéutico, muestra rechazo al uso irracional de antibióticos, pero existe una discrepancia en cuanto a antitusivos y mucolíticos. La aplicación de Nirsevimab en lactantes con comorbilidades presentó alta dispersión (DE = 1,30), lo que sugiere que la estrategia preventiva es de implementación reciente en el país. Conclusión: Es preciso insistir en aspectos relacionados al uso racional de fármacos y prevención de la enfermedad en la comunidad.<hr/>ABSTRACT Introduction: Bronchiolitis is a viral respiratory disease caused by respiratory syncytial virus (RSV), most commonly affecting children under two years of age. Objective: To determine the knowledge and practices of medical students regarding the community-based management and prevention of bronchiolitis in children under two years of age. Materials and Methods: An observational, quantitative, descriptive, cross-sectional study was conducted among 92 fourth- and fifth-year medical students at Universidad Centro Médico Bautista, Asunción, Paraguay, during December 2025 and the first quarter of 2026. Following informed consent, participants completed a self-administered questionnaire via Google Forms using a Likert scale. The students' scores had an overall mean of 3.91 (SD = 0.42; observed range: 2.00-5.00). The internal consistency of the questionnaire, estimated using Cronbach's alpha coefficient in the study sample, was 0.799. Results: The level of knowledge was high across the domains of signs and symptoms, home care, therapeutic support, and prevention. Hygiene and environmental control measures approached unanimous agreement. Regarding therapeutic support, students demonstrated rejection of the irrational use of antibiotics; however, there was some disagreement regarding the use of antitussives and mucolytics. The application of nirsevimab in infants with comorbidities showed substantial variability (SD = 1.30), suggesting that this preventive strategy has only recently been implemented in the country. Conclusion: Greater emphasis is needed on the rational use of medications and the prevention of bronchiolitis in the community. <![CDATA[Microbiological characterization and risk factors associated with coinfections in children under two years of age hospitalized with acute respiratory infections at a referral hospital, january 2023-may 2024]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200172&lng=en&nrm=iso&tlng=en RESUMEN Introducción: Las infecciones respiratorias agudas (IRA) constituyen el grupo de enfermedades más frecuente en la infancia, especialmente en menores de 2 años. Objetivo: Identificar los microorganismos y los factores de riesgo asociados a IRA en menores de 2 años internados en el Hospital Regional de Ciudad del Este durante 2023-2024. Material y Métodos: Estudio cuantitativo, observacional, descriptivo-analítico, retrospectivo y transversal (N = 427). La identificación microbiológica se realizó mediante PCR multiplex en tiempo real. Se aplicó regresión logística univariada para estimar Odds Ratio (OR) con IC 95%. Resultados: Se identificaron microorganismos en 226 pacientes (278 detecciones). El grupo predominante fue 0-6 meses (52,46%). La media de hospitalización fue 5,5 días. Streptococcus pneumoniae fue el agente detectado con mayor frecuencia (27,70%), seguido de rinovirus (17,27%). La asociación VSR + S. pneumoniae mostró el mayor riesgo (OR= 3,94; IC 95%: 2,15-7,22; p &lt; 0,001). Conclusiones: El VSR es un cofactor crítico que incrementa la probabilidad de co-detección bacteriana con una magnitud superior a otros virus, lo que refuerza la necesidad de una vigilancia etiológica y clínica estrecha.<hr/>ABSTRACT Introduction: Acute respiratory infections (ARIs) constitute the most frequent group of diseases in childhood, particularly among children under two years of age. Objective: To identify the microorganisms and risk factors associated with ARIs in children under two years of age hospitalized at Hospital Regional de Ciudad del Este during 2023-2024. Materials and Methods: A quantitative, observational, descriptive-analytical, retrospective, cross-sectional study was conducted (N = 427). Microbiological identification was performed using real-time multiplex PCR. Univariate logistic regression was used to estimate odds ratios (ORs) with 95% confidence intervals (CIs). Results: Microorganisms were identified in 226 patients, with 278 detections. The predominant age group was 0-6 months (52.46%). The mean duration of hospitalization was 5.5 days. Streptococcus pneumoniae was the most frequently detected pathogen (27.70%), followed by rhinovirus (17.27%). The RSV + S. pneumoniae association showed the highest risk (OR = 3.94; 95% CI: 2.15-7.22; p &lt; 0.001). Conclusions: RSV is a critical cofactor that increases the likelihood of bacterial codetection to a greater extent than other viruses, reinforcing the need for close etiological and clinical surveillance. <![CDATA[Experiences and perspectives of parents of children hospitalized in a pediatric intensive care unit at a public hospital in Paraguay]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200182&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La hospitalización de un niño en una unidad de terapia intensiva pediátrica (UTIP) constituye una experiencia altamente estresante para los padres, con repercusiones emocionales, familiares y sociales significativas. Comprender sus vivencias resulta fundamental para fortalecer modelos de atención centrados en la familia y orientar intervenciones de apoyo psicosocial en el ámbito hospitalario. Objetivo: Analizar la experiencia y vivencia de los padres de niños internados en una unidad de terapia intensiva pediátrica de un hospital público de Paraguay durante el año 2024. Materiales y Métodos: Se realizó un estudio cualitativo con diseño fenomenológico de tipo descriptivo-interpretativo. Participaron padres de niños internados en la UTIP de un hospital público de referencia, reclutados entre julio y agosto de 2024. La recolección de datos se efectuó mediante entrevistas en profundidad, individuales y semiestructuradas, adaptadas al idioma de los participantes, hasta alcanzar la saturación teórica de la información. El análisis se realizó mediante análisis de contenido temático, utilizando el software ATLAS.ti versión 24.2.0. Se consideraron criterios de rigor cualitativo como credibilidad, transferibilidad, confiabilidad y confirmabilidad. El estudio contó con aprobación de un comité de ética institucional. Resultados: Las vivencias de los padres se caracterizaron por una elevada carga emocional, predominando manifestaciones de ansiedad, miedo, impotencia y culpa, con mayor intensidad y frecuencia entre las madres. Emergieron dificultades vinculadas a la organización familiar, la situación económica y el ámbito laboral. Se identificó una desigualdad en el acceso al alojamiento hospitalario, con beneficios dirigidos principalmente a las madres, lo que generó sentimientos de exclusión y mayor carga logística y emocional en los padres. El trato del personal de salud fue valorado mayoritariamente de forma positiva, aunque persistieron demandas de mayor información y apoyo psicológico. Conclusión: La experiencia de los padres de niños internados en terapia intensiva pediátrica es compleja y multidimensional, con un impacto significativo en su bienestar emocional. Las inequidades en el apoyo institucional evidencian la necesidad de fortalecer políticas hospitalarias con enfoque de género y atención centrada en la familia.<hr/>ABSTRACT Introduction: The hospitalization of a child in a pediatric intensive care unit (PICU) is a highly stressful experience for parents, with significant emotional, family, and social repercussions. Understanding their experiences is essential for strengthening family-centered models of care and guiding psychosocial support interventions in the hospital setting. Objective: To analyze the experiences and perspectives of parents of children hospitalized in a pediatric intensive care unit at a public hospital in Paraguay during 2024. Materials and Methods: A qualitative study with a descriptive-interpretive phenomenological design was conducted. Participants were parents of children hospitalized in the PICU of a public referral hospital, recruited between July and August 2024. Data were collected through individual, in-depth, semistructured interviews adapted to the participants' language until theoretical saturation was reached. The data were analyzed using thematic content analysis with ATLAS.ti version 24.2.0. Qualitative rigor criteria, including credibility, transferability, dependability, and confirmability, were considered. The study was approved by an institutional ethics committee. Results: Parents' experiences were characterized by a high emotional burden, with anxiety, fear, helplessness, and guilt predominating and occurring with greater intensity and frequency among mothers. Difficulties related to family organization, financial circumstances, and employment were identified. Inequities in access to hospital accommodation were also identified, with benefits primarily directed toward mothers, generating feelings of exclusion and increasing the logistical and emotional burden on fathers. Interactions with healthcare personnel were generally evaluated positively, although participants expressed a need for greater information and psychological support. Conclusion: The experience of parents of children hospitalized in the pediatric intensive care unit is complex and multidimensional, with a significant impact on their emotional well-being. Inequities in institutional support highlight the need to strengthen hospital policies incorporating a gender-sensitive approach and family-centered care. <![CDATA[Early childhood caries and oral health-related quality of life in preschool children in Asunción: a cross-sectional study]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200193&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La caries de infancia temprana (CIT) es uno de los problemas de salud bucal más frecuentes en la población preescolar y puede afectar negativamente la calidad de vida relacionada con salud bucal (CVRSB) de los niños y sus familias. Objetivo: Evaluar la asociación entre la presencia de CIT y la CVRSB en niños de 4 y 5 años que asistieron a instituciones educativas de Asunción durante el año 2024. Materiales y métodos: Estudio transversal realizado en 135 niños preescolares y sus respectivos responsables. La presencia y severidad de caries fueron evaluadas mediante el Sistema Internacional de Detección y Diagnóstico de Caries (ICDAS II), mientras que la CVRSB fue medida mediante la Escala de Impacto de la Salud Oral en la Primera Infancia (ECOHIS). Se aplicaron las pruebas de Chi cuadrado de Pearson, U de Mann-Whitney y H de Kruskal-Wallis. Resultados: La frecuencia de CIT fue del 91,1% y el 56,3% de los niños presentó lesiones severas. Se observó una asociación estadísticamente significativa entre la severidad de caries y los puntajes del ECOHIS (p&lt;0,001), evidenciándose un mayor deterioro de la CVRSB conforme aumentaba la severidad de las lesiones. Los niños con CIT presentaron puntajes ECOHIS significativamente mayores que aquellos sin caries (p&lt;0,001). El dominio más afectado en la sección infantil fue el dolor en dientes, boca o mandíbula, mientras que en la sección familiar fue el tiempo invertido por los cuidadores. Conclusión: La CIT se asoció con una peor CVRSB en los preescolares evaluados, afectando tanto al niño como a su entorno familiar y aumentando su impacto conforme se incrementó la severidad de las lesiones.<hr/>ABSTRACT Introduction: Early childhood caries (ECC) is one of the most common oral health problems among preschool children and can negatively affect oral health-related quality of life (OHRQoL) in children and their families. Objective: To evaluate the association between the presence of ECC and OHRQoL in children aged 4 and 5 years attending educational institutions in Asunción during 2024. Materials and Methods: A cross-sectional study was conducted among 135 preschool children and their respective caregivers. The presence and severity of caries were assessed using the International Caries Detection and Assessment System (ICDAS II), while OHRQoL was measured using the Early Childhood Oral Health Impact Scale (ECOHIS). Pearson's chi-square test, the Mann-Whitney U test, and the Kruskal-Wallis H test were applied. Results: The frequency of ECC was 91.1%, and 56.3% of children had severe lesions. A statistically significant association was observed between caries severity and ECOHIS scores (p &lt; 0.001), with greater deterioration in OHRQoL as lesion severity increased. Children with ECC had significantly higher ECOHIS scores than those without caries (p &lt; 0.001). The most affected domain in the child section was pain in the teeth, mouth, or jaw, whereas in the family section it was the time spent by caregivers. Conclusion: ECC was associated with poorer OHRQoL among the preschool children evaluated, affecting both the children and their families, with greater impact as lesion severity increased. <![CDATA[Foods triggering allergic reactions during complementary feeding: clinical experience at a pediatric referral center in Colombia]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200203&lng=en&nrm=iso&tlng=en RESUMEN Introducción: Las alergias alimentarias pediátricas son un problema creciente de salud pública. Su prevalencia está subestimada debido a la falta de consenso diagnóstico, especialmente durante la alimentación complementaria, etapa crítica para el desarrollo nutricional. Objetivo: Identificar los alimentos locales que generan manifestaciones alérgicas durante la alimentación complementaria en pacientes con diagnóstico de alergia alimentaria. Materiales y Métodos: Estudio observacional descriptivo-retrospectivo basado en la revisión de historias clínicas electrónicas de pacientes de 1 a 36 meses con diagnóstico confirmado de alergia alimentaria, atendidos en el HOMI entre abril de 2022 y abril de 2024. Se excluyeron pacientes oncológicos o con inmunosupresión. Tras aprobación ética, los datos se recolectaron de forma confidencial y se analizaron en STATA 15.0 usando estadística descriptiva. Resultados: Se incluyeron 50 pacientes con edad media de 19,4 meses (DE ±9,5); 56% eran hombres. Las manifestaciones clínicas fueron predominantemente gastrointestinales (61,5%), seguidas de formas mediadas por IgE (30%) y mixtas (8,5%). Los principales alérgenos fueron leche de vaca (61%) y huevo (57%). Durante el seguimiento, el 48% presentó mejoría sintomática y el 22% remisión completa. No se encontraron asociaciones estadísticamente significativas entre niveles de IgE específica y estado nutricional medido por talla/edad (p = 0,957), peso/edad (p = 0,37) y peso/talla (p = 0,803). Conclusión: La leche de vaca y el huevo fueron los principales desencadenantes de alergia durante la alimentación complementaria. No se hallaron asociaciones significativas entre sensibilización (IgE específica) y estado nutricional, lo que sugiere la necesidad de mejorar los registros clínicos para una mejor caracterización y seguimiento.<hr/>ABSTRACT Introduction: Pediatric food allergies are an increasing public health concern. Their prevalence is underestimated due to the lack of diagnostic consensus, particularly during complementary feeding, a critical stage of nutritional development. Objective: To identify local foods that trigger allergic manifestations during complementary feeding in patients with a diagnosis of food allergy. Materials and Methods: A descriptive retrospective observational study was conducted based on a review of electronic medical records of patients aged 1-36 months with a confirmed diagnosis of food allergy who were treated at HOMI between April 2022 and April 2024. Patients with cancer or immunosuppression were excluded. Following ethical approval, data were collected confidentially and analyzed using STATA 15.0 with descriptive statistics. Results: Fifty patients were included, with a mean age of 19.4 months (SD ± 9.5); 56% were male. Clinical manifestations were predominantly gastrointestinal (61.5%), followed by IgE-mediated forms (30%) and mixed forms (8.5%). The main allergens were cow's milk (61%) and egg (57%). During follow-up, 48% of patients showed symptomatic improvement and 22% achieved complete remission. No statistically significant associations were found between specific IgE levels and nutritional status assessed by height-for-age (p = 0.957), weight-for-age (p = 0.37), and weight-for-height (p = 0.803). Conclusion: Cow's milk and egg were the main triggers of allergic reactions during complementary feeding. No significant associations were found between sensitization, as measured by specific IgE, and nutritional status, suggesting the need to improve clinical records for better characterization and follow-up. <![CDATA[Association between gestational weight gain and birth weight in the mother-newborn dyad at referral hospitals in Asunción and Coronel Oviedo]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200216&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La ganancia de peso gestacional (GPG) es un determinante del crecimiento fetal, independientemente del IMC pregestacional. Objetivo: Evaluar la tendencia entre la ganancia de peso gestacional total y semanal y el peso de nacimiento en una cohorte de gestantes paraguayas atendidas en hospitales de referencia. Materiales y métodos: estudio observacional analítico prospectivo con 100 gestantes ≥18 años, embarazo único, seguidas en tres controles prenatales hasta el parto. La GPG total se calculó como la diferencia del peso entre tercer y primer control, la ganancia semanal como la ganancia total dividida semanas transcurridas; ambas se clasificaron en insuficiente, adecuada y excesiva. El peso de nacimiento se analizó como variable continua y categorica. Resultados: La edad materna promedio fue 28,5±6,2 años. La GPG total media fue 8,3±4,6 kg y semanal 0,2±0,1kg. 90% de los recién nacidos fueron de término y 87% presentó peso adecuado, promedio 3342,1±484,6g. El peso al nacer mostró un incremento progresivo según la GPG total (3278,3g, 3292,6g y 3570,0g para ganancia insuficiente, adecuada y excesiva, respectivamente), con diferencia significativa entre ganancia adecuada y excesiva (p=0,044). No se observaron diferencias en GPG semanal. La macrosomía fue más frecuente en el grupo con ganancia excesiva y el análisis lineal evidenció tendencia positiva entre mayor GPG total y mayor peso al nacer (p=0,019). Conclusión: La GPG total mostró una tendencia positiva con el peso de nacimiento, destacando la importancia de monitorizar la ganancia ponderal durante el embarazo y generar evidencia local para optimizar recomendaciones de control prenatal en Paraguay.<hr/>ABSTRACT Introduction: Gestational weight gain (GWG) is a determinant of fetal growth, regardless of pregestational body mass index (BMI). Objective: To evaluate the association between total and weekly gestational weight gain and birth weight in a cohort of Paraguayan pregnant women receiving care at referral hospitals. Materials and Methods: A prospective, analytical observational study was conducted among 100 pregnant women aged ≥18 years with singleton pregnancies who were followed through three prenatal visits until delivery. Total GWG was calculated as the difference in weight between the third and first prenatal visits, while weekly weight gain was calculated by dividing total weight gain by the number of weeks elapsed. Both variables were categorized as insufficient, adequate, or excessive. Birth weight was analyzed as both a continuous and categorical variable. Results: The mean maternal age was 28.5 ± 6.2 years. Mean total GWG was 8.3 ± 4.6 kg, and mean weekly weight gain was 0.2 ± 0.1 kg. Overall, 90% of newborns were born at term, and 87% had an appropriate birth weight, with a mean birth weight of 3342.1 ± 484.6 g. Birth weight increased progressively according to total GWG (3278.3 g, 3292.6 g, and 3570.0 g for insufficient, adequate, and excessive weight gain, respectively), with a statistically significant difference between adequate and excessive weight gain (p = 0.044). No significant differences were observed according to weekly GWG. Macrosomia was more frequent among women with excessive weight gain, and linear analysis demonstrated a positive trend between greater total GWG and higher birth weight (p = 0.019). Objetivo: Determinar factores sociodemográficos, familiares y obstétricos asociados con la lactancia materna exclusiva durante los primeros seis meses de vida. Materiales y Métodos: Estudio observacional, analítico, de corte transversal, en 371 madres de lactantes &lt;2 años atendidos en un hospital de referencia. Se analizaron variables sociodemográficas, familiares, obstétricas y relacionadas con la lactancia materna. Se realizó un análisis bivariado utilizando la prueba de Chi-cuadrado de Pearson y la estimación de riesgos relativos (RR) con sus intervalos de confianza del 95% mediante tablas de contingencia 2×2. El procesamiento y análisis estadístico se realizó en R versión 4.5.1 (R Foundation for Statistical Computing, Viena, Austria). Resultados: La edad media de las madres fue 29±5,6 años y de los lactantes 14±6,1 meses. El 50,1% de las madres tenía empleo remunerado, 93,3% realizó controles prenatales suficientes, 53,1% de los nacimientos ocurrió por cesárea, 74,7% realizó contacto piel con piel y 88,1% inició la lactancia durante la primera hora de vida. El 81,1% mantuvo la lactancia materna exclusiva durante ≥6 meses. En el análisis multivariado, la edad materna ≥20 años, la presencia de una red de apoyo familiar, una jornada laboral ≤6 horas y el inicio de la lactancia en la primera hora se asociaron de forma independiente con el mantenimiento de la lactancia materna exclusiva, mientras que la educación universitaria, el apoyo económico del padre y la ocupación materna como ama de casa no mostraron asociación independiente. Conclusiones: El mantenimiento de la lactancia materna exclusiva se asoció principalmente con factores laborales y el apoyo familiar, destacando la importancia de fortalecer las políticas de protección de la maternidad y las redes de apoyo.<hr/>ABSTRACT Introduction: Exclusive breastfeeding during the first six months of life is a fundamental intervention for child health, and its maintenance may be influenced by various sociodemographic, family, and obstetric factors. Objective: To determine the sociodemographic, family, and obstetric factors associated with exclusive breastfeeding during the first six months of life. Materials and Methods: An observational, analytical, cross-sectional study was conducted among 371 mothers of infants younger than two years who received care at a referral hospital. Sociodemographic, family, obstetric, and breastfeeding-related variables were analyzed. Bivariate analysis was performed using Pearson's chi-square test, and relative risks (RRs) with 95% confidence intervals (CIs) were estimated using 2 × 2 contingency tables. Statistical processing and analysis were performed using R version 4.5.1 (R Foundation for Statistical Computing, Vienna, Austria). Results: The mean maternal age was 29 ± 5.6 years, and the mean age of the infants was 14 ± 6.1 months. Overall, 50.1% of mothers had paid employment, 93.3% had adequate prenatal care, 53.1% of births occurred by cesarean delivery, 74.7% practiced skin-to-skin contact, and 88.1% initiated breastfeeding within the first hour of life. Overall, 81.1% maintained exclusive breastfeeding for ≥6 months. In the multivariable analysis, maternal age ≥20 years, the presence of a family support network, a workday of ≤6 hours, and initiation of breastfeeding within the first hour of life were independently associated with maintenance of exclusive breastfeeding. In contrast, university education, financial support from the father, and maternal occupation as a homemaker were not independently associated with maintenance of exclusive breastfeeding. Conclusions: Maintenance of exclusive breastfeeding was primarily associated with occupational factors and family support, highlighting the importance of strengthening maternity protection policies and support networks. <![CDATA[Rare neonatal arrhythmia: a case report of atrial flutter]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200239&lng=en&nrm=iso&tlng=en RESUMEN Introducción: el flutter auricular neonatal es una arritmia infrecuente pero potencialmente grave; puede detectarse en etapa perinatal y asociarse a compromiso hemodinámico significativo tras el nacimiento. Caso clínico: recién nacido masculino prétermino tardío, con diagnóstico prenatal de flutter auricular e insuficiencia tricuspídea moderada a severa. la madre recibió digoxina antes del parto, que se realizó por cesárea debido a taquicardia fetal. en las primeras horas de vida, el neonato desarrolló dificultad respiratoria e inestabilidad hemodinámica, requiriendo ventilación mecánica y soporte inotrópico. el electrocardiograma confirmó flutter auricular y la ecocardiografía mostró hipocontractilidad miocárdica sin cardiopatía estructural. el tratamiento con amiodarona fue inefectivo, por lo que se indicó cardioversión eléctrica sincronizada, logrando la restitución del ritmo sinusal. la evolución fue favorable y el paciente fue dado de alta con propranolol y seguimiento ambulatorio. Conclusión: la cardioversión eléctrica sincronizada demostró ser un tratamiento seguro y eficaz en el flutter auricular neonatal.<hr/>ABSTRACT Introduction: Neonatal atrial flutter is an uncommon but potentially serious arrhythmia. It may be detected during the perinatal period and may be associated with significant hemodynamic compromise after birth. Case Report: A late-preterm male newborn with a prenatal diagnosis of atrial flutter and moderate-to-severe tricuspid regurgitation was delivered by cesarean section because of fetal tachycardia. The mother had received digoxin before delivery. During the first hours of life, the neonate developed respiratory distress and hemodynamic instability, requiring mechanical ventilation and inotropic support. Electrocardiography confirmed atrial flutter, and echocardiography demonstrated myocardial hypocontractility without structural heart disease. Treatment with amiodarone was ineffective; therefore, synchronized electrical cardioversion was performed, successfully restoring sinus rhythm. The clinical course was favorable, and the patient was discharged on propranolol with outpatient follow-up. Conclusion: Synchronized electrical cardioversion proved to be a safe and effective treatment for neonatal atrial flutter. <![CDATA[Cardiorespiratory arrest potentially associated with ondansetron use in a pediatric patient: a case report]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200244&lng=en&nrm=iso&tlng=en RESUMEN Introducción: El ondansetrón, un antagonista potente y selectivo de los receptores 5 hidroxitriptamina 3, por lo que ha sido utilizado como antiemético, y era considerado como un fármaco seguro por su baja tasa de efectos adversos, sin embargo, algunos estudios han demostrado que puede generar complicaciones severas. El objetivo del manuscrito es presentar el caso clínico de un paciente pediátrico que falleció por complicaciones relacionadas con el uso de ondasentron. Caso clínico: Paciente femenina de 11 meses de edad quien fue llevada por presentar intolerancia a la vía oral, emesis de gasto moderado y episodios diarreicos de 4 días de evolución asociado a deshidratación. Ante esto se decidió iniciar reposición de líquidos endovenosos y adicionalmente se administró única dosis de ondasentron, donde la paciente presentó paro cardiorespiratorio no reversible dos horas después. La autopsia clínica no reportó hallazgos relacionados al fallecimiento, sin embargo, no se descartó una posible arritmia relacionada con la administración del medicamento. Conclusiones: El ondansetrón es eficaz como antiemético y tiene un perfil de seguridad relativamente favorable, sin embargo, se justifica la conciencia de los distintos riesgos de su uso. Varios estudios han reportado que este fármaco presenta cardiotoxicidad, donde se han encontrado asociado a alteraciones electrolíticas, arritmias cardiacas e incluso paradas cardíacas. El caso llama la atención dado que ante un cuadro de diarrea y deshidratación en el que se había logrado controlar los demás factores, se trata de un ddiagnóstico que no es del todo contundente dado que los eventos cardíacos y arritmias suceden durante la administración del medicamento y no es común que suceda horas después. Una anamnesis es importante para estos casos, sin embargo, no hay un factor específico a tener presente, además un ECG inicial normal no garantiza una infusión segura de este fármaco.<hr/>ABSTRACT Introduction: Ondansetron is a potent and selective 5-hydroxytryptamine 3 (5-HT3) receptor antagonist widely used as an antiemetic and has generally been considered a safe medication because of its low rate of adverse effects. However, some studies have demonstrated that it may cause severe complications. The objective of this report is to present the clinical case of a pediatric patient who died from complications potentially related to ondansetron use. Case Report: An 11-month-old female patient was brought for evaluation because of oral intolerance, moderate-volume vomiting, and diarrhea for four days, associated with dehydration. Intravenous fluid replacement was initiated, and a single dose of ondansetron was administered. Two hours later, the patient experienced irreversible cardiorespiratory arrest. Clinical autopsy findings did not reveal abnormalities explaining the death; however, a possible arrhythmia associated with administration of the medication could not be ruled out. Conclusions: Ondansetron is effective as an antiemetic and has a relatively favorable safety profile; nevertheless, awareness of the potential risks associated with its use is warranted. Several studies have reported cardiotoxicity associated with this medication, including electrolyte abnormalities, cardiac arrhythmias, and even cardiac arrest. This case is noteworthy because, in the setting of diarrhea and dehydration in which other contributing factors had been addressed, the diagnosis of a medication-related cardiac event remains inconclusive, given the temporal relationship between medication administration and the subsequent cardiac event. A thorough medical history is important in such cases; however, there is no single specific risk factor that reliably predicts these events, and a normal baseline ECG does not guarantee safe administration of the medication. <![CDATA[Mucopolysaccharidosis type i in a child from the peruvian highlands: clinical consequences of delayed diagnosis without treatment. Se clasifica en tres formas: síndrome de Scheie (leve), Hurler-Scheie (moderada) y Hurler o MPS-IH (severo), esta última con una incidencia de 1 por cada 100 000 nacimientos. Objetivo: Reportar un caso de MPS-IH en un contexto con limitaciones administrativas y políticas en salud. Caso clínico: Paciente masculino de 4 años con diagnóstico confirmado mediante estudio molecular y enzimático. Presentó síntomas progresivos desde los primeros meses, como mancha mongólica extensa, infecciones recurrentes y facies toscas, entre otras. Aunque necesitaba acceder al tratamiento enzimático por donación, no lo recibió debido a barreras administrativas, evolucionando negativamente. Conclusión: Este caso ilustra la progresión severa de MPS I sin tratamiento, agravada por diagnóstico tardío y barreras del sistema de salud. Resalta la urgencia de detección temprana, formación médica y acceso a terapias huérfanas.<hr/>ABSTRACT Introduction: Mucopolysaccharidosis type I (MPS I) is a rare genetic disorder caused by alpha-L-iduronidase deficiency, resulting in systemic accumulation of glycosaminoglycans. It is classified into three forms: Scheie syndrome (mild), Hurler-Scheie syndrome (moderate), and Hurler syndrome or MPS-IH (severe), the latter having an incidence of approximately 1 per 100,000 births. Objective: To report a case of MPS-IH in a setting characterized by administrative and health policy limitations. Case Report: A 4-year-old male patient with a diagnosis confirmed by molecular and enzymatic testing presented with progressive symptoms beginning during the first months of life, including an extensive Mongolian spot, recurrent infections, and coarse facial features, among others. Although he required access to enzyme replacement therapy through a donation program, he did not receive treatment because of administrative barriers and subsequently experienced clinical deterioration. Conclusion: This case illustrates the severe progression of untreated MPS I, exacerbated by delayed diagnosis and healthcare system barriers. It highlights the urgent need for early detection, medical education, and access to orphan therapies. <![CDATA[Surgical management of alveolar clefts in patients with cleft lip and palate: A narrative review]]> http://scielo.iics.una.py/scielo.php?script=sci_arttext&pid=S1683-98032026000200257&lng=en&nrm=iso&tlng=en RESUMEN Un importante número de los pacientes con fisura labiopalatina presentan una discontinuidad del hueso maxilar en la región alveolar, la cual puede manifestarse de manera unilateral o bilateral y se denomina fisura alveolar. Este defecto reviste gran importancia clínica, dado que compromete la erupción dentaria al carecer de un soporte óseo adecuado. Su persistencia se asocia con diversas complicaciones, entre ellas la formación de fístulas oronasales, la aparición de asimetrías faciales, alteraciones en la función fonatoria, así como hipoplasia o ectopia del incisivo lateral. La corrección quirúrgica puede llevarse a cabo en distintos momentos evolutivos y mediante diferentes técnicas; sin embargo, la osteoplastia secundaria con injerto óseo, realizada antes de la erupción del diente adyacente a la hendidura, constituye el procedimiento más empleado. El injerto óseo autólogo es considerado el material de elección por su capacidad de favorecer los procesos de osteogénesis, osteoinducción y osteoconducción. Los sitios donantes más utilizados corresponden a la cresta ilíaca y la región parasinfisaria. El abordaje integral de la fisura alveolar requiere una estrategia multidisciplinaria y la aplicación estricta de los principios terapéuticos en cada etapa del proceso clínico, con el propósito de garantizar resultados funcionales y estéticos óptimos.<hr/>ABSTRACT A significant proportion of patients with cleft lip and palate have a discontinuity of the maxillary bone in the alveolar region, which may be unilateral or bilateral and is referred to as an alveolar cleft. This defect is of considerable clinical importance because it compromises tooth eruption due to the lack of adequate bony support. Persistence of the defect is associated with several complications, including oronasal fistula formation, facial asymmetry, alterations in phonatory function, and hypoplasia or ectopia of the lateral incisor. Surgical correction can be performed at different stages of development using various techniques; however, secondary alveolar bone grafting performed before eruption of the tooth adjacent to the cleft is the most commonly used procedure. Autologous bone grafting is considered the material of choice because of its capacity to promote osteogenesis, osteoinduction, and osteoconduction. The most commonly used donor sites are the iliac crest and the parasymphyseal region. Comprehensive management of alveolar clefts requires a multidisciplinary approach and strict adherence to therapeutic principles at each stage of clinical care, with the goal of achieving optimal functional and aesthetic outcomes.
